PAYMENT AND CANCELLATION POLICY

 

This is a fee-for-service practice and does not accept private insurance. Payment is due at the time of the appointment. Cash and credit cards are accepted.  A receipt for services will be provided upon request. I am currently licensed to treat residents of NY, NJ, and MN only. Please review my Policies and Patient Agreement Form under the “Forms” tab for more practice information.

Currently, all initial consultations are being conducted virtually. Follow-up appointments are available in-person.

I ask that my returning patients let me know about a cancellation at least 24 hours in advance. Electronic appointment reminders are sent two days prior to scheduled appointments. Missed appointments and late cancellations will be charged in full. 

In the event of an emergency, such as a severe allergic reaction or altered mental status, go to your closest emergency room right away or call 911. If you are having a mental health crisis, you can call the National Suicide Prevention Lifeline by dialing 988 and remaining on the line. Please note that I operate this office part-time and cannot offer emergency or after hours coverage.
