TERMS AND CANCELLATION POLICY
Payment is collected at the time of the encounter. Cash and credit cards are accepted. Although I do not accept insurance at this time, most insurance companies reimburse some portion of the cost of out of network services provided that a bill for service is submitted. Bills are generated upon patient request.
Video conferencing is also offered. Regular appointment lengths and fees apply. Please note that video conferencing is generally prioritized for patients with transportation or mobility barriers, such as patients recently discharged from the hospital or a physical rehabilitation facility. The office building is wheelchair accessible otherwise. Please indicate if you require video conferencing for our initial encounter in the “reason for visit” section of the appointment request.
Please also indicate if you would prefer to speak with me prior to our first appointment in the “reason for visit” section of the appointment request. We can speak by phone or by email if you have any specific questions at no additional charge.
Returning patients will be invited to create an account. Although not required, an account will allow us to better coordinate our appointments, share laboratory and imaging results, and streamline payments and prescribing. The prescribing of medications, blood work, and imaging studies is largely done electronically.
Each account is equipped with its own secure patient portal powered by Kareo where direct messaging with me is available. The portal messaging service is designed to address questions or concerns in between appointments, such as medication clarifications or to provide notice of cancellation. The messaging service is not intended to replace face to face appointments in the event of a cancellation or no show or to post about emergent matters.
In the event of an emergency, such as severe allergic reactions or suicidal thoughts, please go to your closest emergency room and then provide your ER team with my information so that we may collaborate regarding your care.
Misuse of the patient portal, specifically the posting of inflammatory or threatening material, is grounds for discharge from the practice. Overuse of the portal messaging service (more than 3 postings between appointments on more than one occasion) cannot be accommodated.
I ask that my returning patients let me know about a cancellation at least 24 hours in advance. Appointments cancelled later than that will be charged in full. No shows are similarly charged in full.
To avoid this, however, electronic appointment reminders will be sent one week and again 24 hours prior to appointments, at which times cancellations are free of charge. Appointment reminders are sent via text message and email. Please notify me of a cancellation either by phone, email, or on the portal messenger.
